If anyone wanting cover has a pre-existing medical condition(s) or any other query regarding  medical cover on this policy  then they must contact Healthcheck Screening, tel. 08445 - 449915,  Monday - Fri. 9am - 5pm  to find out if they are willing to cover the condition(s) and how much extra you would need to make as a separate extra payment made direct to Healthcheck. You must also take out your policy with Holiday Services by printing off and completing the proposal form ( see link for this form further down this webpage. ) If Healthcheck are unable to offer cover for your conditions(s) or you consider that the extra payment required is excessive contact Mike on 01773 - 747406 ( 747426 ) as we may be able to offer alternative schemes.
